Most bilingual and multilingual countries have two or more official languages, and in such countries all children might be required to learn a second language. The most speakers of English who speak it as their second language do so with a dialect is readily understood. Many ethnocultural group in the U.S. speak English as a second language. A secondary English dialect is a variation of any one form of native English, spoken by a bilingual or multilingual individual, and is largely influenced by the individual's primary language, which is something other than English. Dialects of English that are learned and used in a speaker's second language are described as secondary. Dialects of a language are a strong reminder that general rules of a language do not produce a uniform method of speaking a language. Consequently, English has many dialects, some primary and some secondary or non-native. It is spoken in many countries, both as a primary language and as a second language. That English is more international in its scope than other language is well known. On the other hand, dialect is a broader concept that includes phonologic, morphologic, semantic, and pragmatic differences in verbal practices of subgroups of a community. Accent is restricted to speech sound production differences and different rhythms of speech. Variations of a specific language are called its dialects, which should be distinguished from accents. Because the majority of the population speaks English, people who speak other languages are considered the language minority. A country or society of differing cultural practices is multicultural, and practically all societies are multicultural. People who speak two languages are bilingual, and those who speak more than two languages are multilingual. A person's voice, articulation, and language may be within the normal limits but still may be vulnerable to problems of communication known as fluency disorders. Some people may still believe that the U.S. is a unilingual English-speaking country partly because the country has a single federal official language and because the majority of people speak only English. In fact, multiculturalism and multilingualism go together. Historically, it has been believed that different people who immigrate to the U.S. lose much of their cultural practices and evolve into a single cultural group in this 'melting pot'. Diversity may be marked in countries with different languages or historically isolated geographic regions. Nonetheless, most of the countries of the world are multicultural. Children who have normal hearing sensitivity associated with aging also may be sensorineural. If the delivery of a baby is prolonged and distressed, the baby may suffer from anoxia, which can damage the hearing mechanism and cause sensorineural hearing loss. In sensorineural hearing loss, the inner ear, the auditory nerve that transmits sound to the brain, or both may be affected. Conductive hearing loss also may be due to otosclerosis, in which the tiny bones of the middle ear do not vibrate normally because of soft, spongy growth on them. A frequent cause of conductive hearing loss in children is otitis media, or middle ear infection, due to cold, allergy, and other reasons. In conductive hearing loss, the sound transmission from the outer or middle ear to the inner ear is impaired. Stuttering is a disorder of fluency with excessive amounts or excessively long durations of dysfluency, which often are combined with tension, struggle, and related behaviors. Cluttering involves impaired fluency and rapid but disordered articulation, possibly combined with disorganized thought. The person who has a fluency disorder knows exactly what to say and how to put it but cannot say it fluently.
