Voice therapy is preferable to surgical removal of vocal nodules. Such biofeedback instruments can be used in teaching the client to relax the speech muscles. The relative electrical activity of the muscles is an indication of relative tension. With the help of surface electrodes fastened onto the skin, the unit can record electrical activity of the muscles. For instance, a client's shallow breathing may require frequent inhalation that interrupt voice and speech. Essentially, the voice evaluation includes making measurements and clinical judgments regarding the client's breathy supply, laryngeal tension, voice quality, vocal pitch and loudness, resonance, situational variability and types and frequency of vocally abusive behaviors. After taking the case history, the clinician may complete the orofacial examination to make sure there are no gross organic defects in the oral region. The history will also document previous medical or voice treatment and their outcome, the reason for the current referral, and the general health of the client. The speech-language pathologist makes an evaluation of the voice and its qualities, how the client is using his or her voice, the life conditions affecting the client's voice and its production, and the kinds of behavioral patterns related to disordered voice. It is the responsibility of the speech-language pathologist to change the vocally abusive behaviors that have caused the nodules and the resulting voice problem. Vocal rest and voice therapy usually clear the nodules, but prolonged abuse of the folds results in larger and more persistent nodules. See Textbox 8.4 for the story of a famous singer whose surgical removal of her vocal nodules negatively affected her singing career. Some singer's careers have been cut short by surgery for the nodules. Speech clinicians are in a position to notice these early signs of papillomas and refer the patients to medical specialists. Because the vocal nodules make it difficult to close the folds firmly, air leaks during phonation, causing a breathy voice. Any profession that requires a constant and intense use of voice poses a risk for vocal nodules. Such friction is necessary to produce the screams of joy and shouts of contempt. They are well defined in the beginning stages but become more diffuse later because of the inflammation of the surrounding tissue. The vocal nodules frequently appear at the junction of anterior and middle-third portion of the vocal folds. The vocal fold thickening can be reduced by eliminating the vocally abusive behaviors. The speech-language pathologist, however, must help the client change the faulty methods of voice initiation and control excessive talking. The resulting laryngeal trauma is thought to be the cause of contact ulcers. A person with contact ulcers likely talks excessively. Voice therapy does not prevent future occurrences of the papillomas, but it can help the patient make the best use of his or her weakened and scarred vocal mechanism. Repeated operations weaken the vocal folds and create scar tissues that affect the patient's voice. Some children are reported to have had 15 or more operations within a few months. The papillomas tend to recur, creating a need for repeated surgery. Surgery may permanently scar the vocal folds, affecting its efficiency.
