papilloma
One kind of growth on the laryngeal structures, including the vocal folds, is called papilloma. Some papillomas or papillomata are hard and look like warts. The vocal folds can be covered with such wart-like growth. Other papillomas are soft and do not resemble warts. Papillomas are found more frequently in children than in adults. Children born to mother who have genital warts may have a papilloma of the larynx at the time of birth. Papillomas are not the same as carcinoma, but often a biopsy is needed to distinguish them. Papillomas are thought to be caused by a virus, but this is not known for sure. Papillomas can create serious breathing problems because they tend to grow quickly and occupy much of the glottal area. Therefore, papillomas can be life threatening. The patient's voice sounds breathy and hoarse. Speech clinicians are in a position to notice these early signs of papillomas and refer the patients to medical specialists. Papillomas are typically treated surgically. The traditional surgical method is to excise the wart-like growths. Ultrasonic and laser surgeries are also available. Unfortunately, the papillomas tend to recur, creating a need for repeated surgery. Some children are reported to have had 15 or mor operations within a few months. Repeated operations weaken the vocal folds and create scar tissues that affect the patient's voice. However, a vaccine developed from a sample of the papilloma may prevent the recurrence of the disease. After the surgical treatment, voice therapy can help many patients. Voice therapy does not prevent future occurrences of the papillomas, but it can help the patient make the best use of his or her weakened and scarred vocal mechanism.
