Individuals with right hemisphere disorder may experience emotions as usual, but they may have difficulty expressing them and understanding emotions when others express them. They find it difficult to express their emotions through their tone of voice and facial expression. For example, instead of expressing their anger through their tone of voice, they may say, "I am angry at you.", suggesting that they do experience the emotion and perhaps even appreciate that their tone of voice gives no indication of their emotional state. Therefore, predictably, comprehending emotions that other people convey through their facial expressions and tone of voice may be especially for people with right hemisphere disorder. The individuals also may be confused about emotions expressed in printed pictures or sentences. Reasoning, planning, organizing, and problem-solving skills may all be affected in individuals with right hemisphere disorder. It may be difficult to plan a vacation, meal, or party. Writing an organized shopping list may be problematic. It may be difficult to infer the immediate past or future events of a currently depicted situation. For example, when they are looking at a dog shaking water off its body, they may not infer that the dog got wet. When presented with absurd statements, they not only may fail to see the absurdity but also may justify it with equally absurd reasons. Pure speech and language deficits are less common in right hemisphere damage than they are in left hemisphere damage. Word-finding problems, agrammatical speech, paraphasia, and circumlocution that characterize left hemisphere injury are not dominant in right hemisphere injury. The impaired communication found in person with right hemisphere disorder is mostly due to their attention deficits, disorientation, and left neglect. For example, the individual may give inappropriate or anomalous verbal responses, possibly due to lack of attention to what others ask or say. Unlike those with left hemisphere injury, those with right hemisphere injury may be inappropriately humorous, confabulate, and ramble when they talk. They often have difficulty distinguishing significant from irrelevant information. For example, when telling stories, they may concentrate on irrelevant details or dwell on minor points while missing the most important points. Individuals with right hemisphere disorder may fail to grasp abstract, alternative, or implied meanings of messages. These individuals tend to concentrate only on the concrete or literal meanings. Proverbs, idioms, and such other abstract language expressions may be especially difficult for them to understand or interpret. Ironic, sarcastic, or humorous statements may pose similar problems for them. They may give the names of individual items in a category, but fail to give the category name. Conversational skills may be significantly affected in individuals with right hemisphere disorder. For instance, staying on a topic of conversation and talking appropriate conversational turns may be impaired. Eye contact may not be maintained, and responses may be impulsive. Individuals may not rephrase what they said when their listeners fail to understand them. Additional communicative problems may include difficulties in auditory comprehension of spoken language and problem reading and writing. Because of their left neglect, individuals may ignore the left part of the page while reading. They may then complain that what they read makes no sense. As they write or draw, they may neglect the left side of the page and crowd letters and drawings into the right side.
