A majority of individuals who need long-term rehabilitation are those who receive non-penetrating brain injuries. Penetrating brain injuries create acute medical conditions but not necessarily a need for long-term rehabilitation. Therefore, much effort in the rehabilitation of individuals with traumatic brain injury is directed toward non-penetrating injuries. Non-penetrating traumatic brain injury causes a sequence of neuropathological events. Blood may accumulate in the brain or within the different meningeal layers, causing death of brain tissue. The pressure within the skull may increase, partly due to swelling of the brain tissue. Breathing difficulties may limit oxygen to the brain, also causing the death of brain tissue. Some persons develop seizures. Other effects of traumatic brain injury are varied. Depending on the severity of traumatic brain injury, a person may be simply dazed and inattentive to stimuli, or he or she may enter a state of stupor or coma. In a state of stupor, the person is unresponsive to anything except for pain or other strong stimuli. In a state of coma, the person is unconscious and unresponsive to most stimuli and may die without regaining consciousness. Others may remain in a vegetative state for long periods. Nearly all are confused, inconsistent, disorganized, disoriented to time and place, distracted, restless, and irritated. Impaired memory and reasoning skills are typical of individuals with traumatic brain injury. Swallowing disorders also are a consequence of traumatic brain injury in some cases. Some experience hallucinations and delusions. Apathy and depression also have been associated with traumatic brain injury. Communicative deficits associated with traumatic brain injury are not purely linguistic. Language problems are not as severe as other kinds of communicative problems. For example, grammar may be intact.
