Imagine what happened to a pilot and co-pilot in January 1982 in Washington.
Air Florida Flight 90 had taken off, but it could not keep ascending and crashed into a bridge followed by the Potomac river.
Only 5 out of the 79 people on the plane survived.

How was this mistake made?
The black box conversation revealed that while the pilot had not flown much in icy weather, the co-pilot knew more and had attempted to warn the pilot but to no avail.
The co-pilot thought that the pilot understood certain aspects of flying in icy weather, such as that a tail wind causes speed, but slow flaps would not be enough for a safe landing.
The pilot should then choose not to land.
However, the pilot didn't translate the co-pilot's indirect words as meaning they were traveling too fast.
A linguist for the Institute for Research on Learning in Palo Alto, California, Charlotte Linde, has studied the cockpit conversations that are recorded in black boxes for crashes as well as problematic flight simulations.
She found that it was common that co-pilots, and other subordinates in a crew, would adopt indirect language.
Additionally, the times when indirect language was employed brought more failure than would otherwise happen.
Captains were more likely to ignore the warnings of their subordinates also.
The conclusion is that indirect language is harder to detect and easier to ignore.
Returning to the Air Florida case, while the co-pilot did clearly suggest checking for ice again, the fact that he suggested it rather than commanded it meant the captain could ignore it more easily.
Linde's research has prompted some airlines to train crew to speak up to superiors more forcefully when they are worried about something.
