For the individual to improve vocal loudness, increase sentence lengths, and promote continuous speech, respiratory problems should be reduced. Without a sustained air supply from the lungs, sustained speech is not possible. In treatment sessions, the clinician trains the individual to build up more air pressure below the vocal folds, inhale more deeply before speaking, and exhale in a controlled manner while speaking to produce longer and louder utterances. Pausing at appropriate junctures in speech and breathing again to have sufficient air supply may be taught. Prolonging vowels to progressively greater extents to produce sustained phonation will be helpful. In small and carefully planned steps, the person may be taught to speak in progressively longer sentences. Modification of phonation is needed to improve voice production, encourage louder speech, and reinforce longer utterances. Several procedures are helpful in eliminating the common voice problems associated with dysarthria. While SLPs use behavioral methods to improve voice production, some medical and surgical procedures also are available. During behavioral training, the clinician may use computerized biofeedback instruments to practice and reinforce progressively louder and sustained voice. In the absence of such instruments, the clinician can model, evoke, and positively reinforce the louder and sustained voice. Phonating while pushing, lifting, or pulling is also practiced to initially increase the vocal intensity. Some persons produce better voice when they tilt their head to one or the other side, and if this is the case, such postural modifications are reinforced. Modification of resonance problems typically consists of reducing hypernasality. Note that hypernasality is a result inadequate closure of the velopharyngeal. Generally, the treatment includes both behavioural training to achieve the velopharyngeal closure and prosthetic approaches. A simple procedure that SLPs use involves a mirror being held against the nostrils to show that excessive air is escaping through the nose during speech production, causing hypernasality. The mirror fogs, indicating nasality. Some individuals may be helped by teaching them to open their mouths wide during speaking, thus directing more of the phonated sound through the mouth, thereby increasing oral resonance and decreasing hyperreality.
