Treatment of individuals with RHD targets the impaired skills, which are grouped for intervention. For instance, one group of skills targeted for treatment may involve impaired attention and left neglect, and another group may consist of appropriate social communication. Although they tend to concentrate on improving social communication skills, to be effective during treatment, speech-language pathologists also need to work on the client's attention, orientation, and left neglect. In some cases, communication intervention may be delayed until the client's neurological status improves and, with it, general attention and orientation. Because many of the problems in communication may arise from fluctuating attention, clinicians in most cases design procedures to improve attention. Clinicians generally work on attention during communication training itself. For instance, clinicians may ask the individual to talk on a specific topic and then stop talking when speech departs from the topic. Frequently, the clinician verbally reinforces continued conversation on the same topic. Such strategies help the individual pay sustained attention to the topic at hand. Similarly, the clinician may frequently prompt the person to maintain eye contact throughout the treatment session and positively reinforce eye contacts. To control impulsive, irrelevant or inappropriate response to stimuli, the clinician alerts the person before showing pictures or other kinds of stimuli. Occupational therapists often treat visual neglect in the context of such daily activities as dressing, grooming, eating, and walking. SLPs also address this problem during communication training and literacy skill retraining. The clinician may force the client's attention to the left side of the page while reading a book; the client may be given such prompts as "Look to the left" when he or she comes to the end of a printed line. A brightly colored border drawn on the left side of printed may also draw attention to the neglected left side of printed material.
